Storytelling Driven by Data

Long Term Care

In today’s digital landscape, data is everywhere, and for assisted living communities and skilled nursing facilities, harnessing the power of data is more critical than ever. Instead of allowing outside voices to dominate the narrative, facilities can proactively present accurate information about quality of care, specialized programs, and resident satisfaction. By paying close attention to this information, long-term care workers can gain insights into what they’re doing well and where they can improve.

This is where the LTC Data Cooperative comes in. Understanding your facility’s data in collaboration with the LTC Data Cooperative can be a tool to help drive meaningful progress. Partnering with the Cooperative provides your team with robust LTC Data Cooperative Provider Reports, generated directly from your Electronic Health Records. These reports detail key information such as hospitalizations by day of the week, vaccination rates across multiple vaccine types, and proportion of residents with advance directive orders, while comparing trends with other Cooperative participants. Leaders of skilled nursing facilities (SNFs) and assisted living communities can share these reports with staff as a starting point for discussions about what factors are affecting resident outcomes. Equipped with these insights, your staff can celebrate successes and proactively refine care practices.

By aggregating data from multiple facilities and partnering with academic researchers, the Cooperative can also illuminate the real-world challenges and successes of the long-term care industry—helping shape policy changes and create broader awareness of quality care. Contributing your facility’s data ensures a more accurate, collective understanding of best practices, care outcomes, and resident needs.

Understanding and leveraging your data allows you to control your story. Data-driven storytelling provides transparency that helps build trust with residents, families, and the wider community. With the right data systems in place, you’ll remain ready to adapt, address issues, and consistently showcase the strengths of your facilities and communities.
