​“In 2022 we began reviewing processes in place and looking for ways to improve. We began a weekly Performance Improvement (PI) meeting. PI meetings are a time for key staff to come together with our medical director and review multiple areas that we know we need to improve on. During PI we review all new admissions. Therapy joins us and gives updates for all of our residents receiving both Part A and Part B services, which helps us to better plan for discharges. Our MDS Nurse reports on falls, which led us to developing a Falls Review Team and we have seen a decrease in falls as a results. Nursing goes over medication changes particularly when it comes to antidepressant and antipsychotic meds. Our Infection Preventionist goes over antibiotic use, UTIs, and isolations. Wound Care Nurse gives an update each week on the size of wounds and treatments. Dietary reports on weight loss and dietary changes and/or needs which led to the development of a Weights Team Meeting to better prevent weight loss. As you can see, PI is truly a team effort, and it allows us to better evaluate weekly so that we have a better grasp on what we need to measure as a part of QAPI (Quality Assurance and Performance Improvement). The feedback that we received on our previous Silver Award made us aware that we needed to have a better system for measuring outcomes and PI is one of the tools that we put into place as a result."
