CMS; Medicare; Skilled Nursing Facilities (SNF)

The Centers for Medicare and Medicaid Services (CMS) has updated information on provider enrollment data for all currently Medicare-enrolled Skilled Nursing Facilities (SNFs) on Data.CMS.gov to include new data fields associated with Private Equity and Real Estate Investment Trust ownership, as well as Additional Disclosable Parties (ADPs). SNF providers provide this data to CMS through Medicare provider enrollment processes, which includes the reporting of specific information on Form CMS 855A used for all new enrollments, reinstatements, change of ownership, revalidation, and change of information transactions.

This data was extracted from the CMS Provider Enrollment, Chain, and Ownership System. Starting in October 2024, SNF providers are required to submit additional information related to ADPs in Attachment 1 on the Form CMS-855A, but have until May 1, 2025, to report the additional information as requested by their respective Medicare Administrative Contractor. Sub-regulatory guidance from CMS regarding the submission of this new information is available here.
